Support Coordination is all about helping you get the most out of your NDIS plan. At Blossom Support Coordination, we work alongside participants and their families to make navigating the NDIS easier, more understandable, and tailored to your individual goals.
Understanding your NDIS plan

We help you make sense of your NDIS plan by breaking down exactly what funding you have and what it can be used for. This includes explaining the different support categories, clarifying the types of services that can be accessed, and showing how to allocate your funding effectively to meet your unique needs. Our goal is to ensure you feel confident making decisions about your supports and using your plan in a way that best helps you achieve your goals.
2| Connecting with services and
providers
Support coordinators assist participants to identify, engage, and connect with suitable NDIS-registered and non-registered providers. This includes allied health professionals, support services, and community-based supports that align with the participant’s needs, goals, and preferences.
3| Coordinating Supports and
Services
Support coordination helps organise and coordinate multiple supports/ stakeholders in your team to ensure services are delivered effectively, everyone collaborates and work together. This reduces duplication, minimises gaps in service delivery, and supports continuity of care.
4| Capacity Building and Skill Development
Support coordination focuses on building a participant’s capacity to manage their supports over time. This may include developing skills in decision-making, understanding service agreements, communicating with providers, and navigating the NDIS system independently.
Support coordinators are here to set you up to manage your plan with independence in the future.
Support coordinators assist participants with plan reviews, change of circumstance requests, and the provision of supporting information where required. This helps ensure supports remain aligned with the participant’s current needs and circumstances.

Support coordination involves monitoring the effectiveness of supports, identifying issues early, and addressing concerns as they arise. Support coordinators may advocate on behalf of participants to resolve service issues and ensure funded supports are delivered as intended.
